All consultations are by Triage Appointment Form
Our appointments are offered to you by using our 'Online Triage' form. This form can be filled in between the hours of 8am and 6:30pm. We have appointments available all day so there is no need to rush at 8am. Please fill in the form in as much detail as possible. This will help the GP to establish the correct/best course of action. All Triage Appointment Forms are sent straight to the GP or our Advanced Nurse practitioner who will respond to you directly via a text message or phone call. Your appointment or outcome will then be resolved effectively and as quickly as possible. We aim to respond within 2 hours.
If you feel that your problem is more urgent or you need help with the form, please contact our reception team who will be happy to facilitate this.
To help us to ensure that the system runs smoothly it would be appreciated if you:

Referrals
When referring you to a consultant, we will hopefully be able to give you an idea of waiting list times. However if you choose to see a consultant privatley we will provide you with a covering letter.
Dial-a-Doc
This service allows you to have a telephone appointment with the doctor instead of having to visit the surgery if appropriate.
Cancelling or changing an appointment
If you can not make your appointment let us know as soon as you can so another patient can book the appointment time. You can cancel or change your appointment:
If you need help when we are closed
- Use NHS 111 online (111.nhs.uk) or call 111. 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
- Out of hours information
- If it’s a life-threatening emergency call 999.

Home Visits
If you are unable to attend the surgery, please inform us as soon as possible. Preferably, we would rather see you in the surgery where there are all the necessary facilities to fully assess your needs.
